Week 12:

Achieving Long-Term Diabetes Management and Health

You have made it to the last week of the I & O Healthcare Diabetes Education and Prevention Challenge! Over the past 12 weeks, you have shown dedication and commitment to improving your health and managing your diabetes. Here’s a look back at what we’ve accomplished together and what resources are available to continue your journey.

Achieving Long-Term Diabetes Management and Health

Managing diabetes is a lifelong commitment that requires continuous effort, learning, and adaptation. As we conclude this challenge, it's essential to focus on strategies and resources that will support your long-term health. Here’s how you can continue to achieve long-term diabetes management and health:

Maintain a Balanced Diet

  • Continue to follow the nutritional principles you’ve learned. Aim for a balanced diet rich in vegetables, whole grains, lean proteins, and healthy fats.

  • Monitor your carbohydrate intake and choose foods with a low glycemic index to maintain stable blood sugar levels.

Stay Physically Active

  • Incorporate regular physical activity into your daily routine. Aim for at least 150 minutes of moderate-intensity exercise per week, such as brisk walking, swimming, or cycling.

  • Include strength training exercises at least two days a week to help manage blood sugar levels and improve overall health.

Monitor Your Blood Sugar

  • Regularly monitor your blood sugar levels as advised by your healthcare provider. Keeping track of your levels helps you understand how different foods, activities, and medications affect your blood sugar.

  • Use your monitoring results to make informed decisions about your diet, exercise, and medication.

Take Medications as Prescribed

  • Follow your healthcare provider's instructions regarding your diabetes medications. Do not skip doses, and ensure you understand how each medication works.

  • Keep an open line of communication with your healthcare provider about any side effects or concerns you may have.

Manage Stress

  • Practice stress-reducing techniques such as yoga, meditation, deep breathing exercises, or hobbies that you enjoy.

  • Seek support from friends, family, or professional counselors if you feel overwhelmed or stressed.

Regular Health Check-Ups

  • Schedule regular appointments with your healthcare provider to monitor your diabetes management and overall health.

  • Keep up with routine screenings and tests, such as A1c tests, blood pressure checks, and eye exams, to catch and address any issues early.

Stay Educated and Informed

  • Continuously educate yourself about diabetes management. Stay updated on new research, treatments, and strategies to manage your condition effectively.

  • Participate in diabetes education programs and workshops to refresh your knowledge and learn new skills.
